• Free preventive care, including 2 check-ups and cleanings per year, a set of x-rays, and 2 fluoride treatments for children 13 and younger

How It Works

When you enroll in the EFD Benefit Plan, you will receive the benefits for 12 months, and payment is due within the first 6.
